PROCESS

1. Confirm that you meet the requirements.

2. Determine a date for each phase.
    (Check our online calendar for possible dates)

3. Fill out our online request form. 
    (Click here for the form)  

4. You will receive a confirmation email.
    (Your request has been approved)

5. Follow up contact prior to the event.
    (7-10 days prior to the event you will be     contacted by phone)

6. Once the event has been completed you      will be asked to take a online survey
